5 Wegovy Symptoms You Should Never Ignore

Wegovy has helped millions of Americans lose weight, but it carries real medical risks that don’t always get enough attention. Most people focus on before-and-after photos and miss the warning signs hiding in plain sight.

Not every side effect is a reason to panic, but some signal something serious. Here are five symptoms on Wegovy that deserve immediate medical attention.

1. Nausea that won’t let up – even after weeks

Nausea is common with Wegovy, particularly in the early weeks after a dose increase. Knowing how long on average does Wegovy nausea last helps you distinguish routine discomfort from something that warrants a call to your doctor. Most mild nausea fades within four to six weeks at any given dose level. When it doesn’t – or when it’s bad enough that you can’t keep fluids down – that’s a different situation entirely.

Uncontrolled vomiting leads to dehydration faster than most people expect. Signs of dehydration include:

  • Dry mouth and dark urine
  • Dizziness or light-headedness when you stand up
  • Decreased urination over a 12-hour period
  • Rapid heart rate at rest

Severe dehydration puts serious strain on your kidneys. Because Wegovy already slows gastric emptying, an inability to absorb any fluids at all may mean you need IV hydration. Don’t wait for it to pass on its own when these signs show up together.

2. Sudden, severe abdominal pain

Sharp, persistent pain in the upper abdomen – especially pain that radiates into your back – is one of the most urgent symptoms associated with GLP-1 receptor agonists like semaglutide. This pattern is consistent with pancreatitis, an inflammation of the pancreas that requires prompt medical evaluation.

Clinical trial data submitted to the FDA flagged cases of acute pancreatitis among people taking semaglutide. The absolute risk is low, but a missed or delayed diagnosis can carry serious consequences. Your pancreas sits just behind your stomach, so the pain can feel deceptively close to an upset stomach or indigestion at first.

The difference matters. Pancreatitis pain typically:

  • Builds steadily over hours rather than coming in waves
  • Gets worse after eating or drinking
  • Comes with nausea, vomiting, and fever
  • Doesn’t respond to antacids or position changes

Go to an emergency room rather than scheduling a routine appointment. Blood tests measuring amylase and lipase levels can confirm or rule out pancreatitis quickly.

3. Changes in your vision

Wegovy and other GLP-1 medications have been linked to a condition called diabetic retinopathy progression, particularly in people who already have some degree of retinal disease. A 2023 observational analysis published in JAMA Ophthalmology identified a higher rate of non-arteritic anterior ischemic optic neuropathy – a form of optic nerve damage – among semaglutide users compared to other weight-loss medication users.

Rapid glucose reduction and blood pressure shifts during the early weeks of treatment can stress small blood vessels in the eye. If you notice any of the following, contact an eye specialist without delay:

  • Blurred or distorted central vision
  • Sudden floaters or flashes of light
  • Partial loss of vision in one or both eyes
  • Colours appearing washed out or dull

You don’t need a pre-existing eye condition for this to become a problem. Any Wegovy user who notices unexplained visual changes should get a dilated eye exam. Catching retinal issues early makes a real difference in outcomes.

4. A resting heart rate that climbs

Wegovy is tied to a small but real rise in resting heart rate. In the STEP clinical trials, participants taking semaglutide 2.4 mg saw an average increase of roughly two beats per minute. That sounds minor, but for some people the rise is larger and comes with noticeable symptoms.

Watch for a resting heart rate consistently above 100 beats per minute when you’re sitting quietly. You might also notice:

  • Palpitations – a fluttering or pounding sensation in your chest
  • Shortness of breath during activities that didn’t bother you before
  • Fatigue that feels disproportionate to your activity level

People who already have a history of arrhythmia, heart failure, or structural heart disease face additional considerations. Wegovy is not approved for use in people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, and any new cardiac symptom warrants a call to your prescribing doctor the same day it starts. Don’t dismiss persistent palpitations as anxiety.

5. Mood shifts, depression, or thoughts of self-harm

The FDA issued a monitoring notice for GLP-1 receptor agonists after post-market reports linked them to suicidal ideation and self-harm behaviours. A definitive causal connection hasn’t been firmly established in large-scale trials, yet both the FDA and the European Medicines Agency conducted safety reviews in 2023 and are still tracking this signal. This symptom category allows for no acceptable “wait and see” approach.

It’s also true that obesity and type 2 diabetes carry elevated baseline rates of depression. Sorting out what’s the medication versus what’s a pre-existing condition isn’t something you should try to do alone. Tell your prescribing provider immediately if you experience:

  • New or worsening depression
  • Feelings of hopelessness or emptiness that appear after starting or increasing your dose
  • Thoughts of harming yourself
  • Unusual irritability or emotional numbness

If you’re in crisis right now, call or text 988 (the Suicide and Crisis Lifeline) or go to your nearest emergency room. Your prescriber may pause or stop Wegovy while working through these symptoms. Weight loss isn’t worth a mental health emergency.
